In honor of National Health Center Week (NHCW), eClinicalWorks wants to recognize a health center that represents this year’s theme, “Celebrating America’s Health Centers: Transforming Health Care in Our Local Communities.” Hometown Health Center (HHC) demonstrates innovation and dedication, ensuring patients receive the utmost care.
Accountable Care Growing Fast
eClinicalWorks recently conducted a survey with responses from 2,100 healthcare professionals in the United States, nearly 700 of them physicians, highlighting motivators, challenges and tools for those pursuing Accountable Care Organizations (ACOs), Patient-Centered Medical Homes (PCMHs) and other risk-based or shared savings programs. More than half (53%) of respondents currently participate in one of the referenced programs. Among those who do not, approximately one-third (34%) anticipate their organization becoming part of an ACO, PCMH or shared-savings plan in the next three years.
Harmonizing University Health
How can universities connect Student Health Services, Registrar’s Office, Athletic Departments and students to coordinate care? How can universities manage health conditions and chronic diseases, reduce the risk of individual illness and injury, assist students in embracing healthy lifestyle choices, and also improve health efficiency?
What the Doctor Ordered: Apps
Patient engagement aims to enhance communication channels between patients and healthcare providers, improving health outcomes and aid care coordination efforts, including Patient-centered Medical Homes (PCHM), Accountable Care Organizations (ACOs) and Meaningful Use. In fact, ONC has released the final Stage 2 criteria for Meaningful Use, which includes requirements for providers to offer online access to health information and secure messaging for 5% of patients, encouraging physicians to implement patient engagement tools.
Driving Via a Rear-view Mirror
As various options for healthcare reform are considered, the patient-centered care model and shared savings programs have come about as alternative approaches to care delivery, performance measurement and value-based payment reform, shifting the focus to preventative care. The pay-per-service model is viewed by some as being similar to driving through the rear-view mirror. What is behind you provides insight, but it is what is in front that is really telling.
